FCHP operates one of the few clinics in Southwest Florida that utilizes General Practitioners and Nurse Practitioners in a collaborative environment. Using a treatment plan combining telehealth and in-person visits creates a hybrid comprehensive care approach.
FCHP came into existence through the collaboration of several healthcare workers who shared a common vision around providing better healthcare solutions to the community. The founding shareholders were passionate about differentiating health care services through innovative solutions to improve patient care quality, improve community health services, and address the ever-increasing challenges around a growing and aging population.
